The Optional Child Life Insurance provides the opportunity to purchase $10,000 or $20,000 for each dependent child. Purchase of $10,000 or $20,000 can be made during the guarantee issue period without evidence of insurability. The rates for the Optional Child Life Insurance are $1 per $10,000 per month. Please note separate elections for each child is not necessary, one election will cover all dependent children. Benefits for infants under 6 months are limited to $1,000 in coverage.

If you are eligible and would like to enroll, complete a Benefits Enrollment Form and return it to HRM/Benefits at 250 Columbus Place. If you are enrolling after the guarantee issue period, you must complete an Evidence of Insurability Form and submit it to HRM/Benefits at 250 Columbus Place.
